Transition Care Telemetry offers a broad range of hospice and palliative care services. We take care of medications, medical equipment, and bereavement services for patients and their families. We are available 24/7 – including weekends and holidays – to take care of your loved ones and help answer any of your questions. We believe that hospice is not about giving up on life; rather, it is about living life to its fullest. We help by providing pain and symptom management, comfort, and companionship.
Expert medical, emotional, and physical care
It’s not easy once a loved one has passed away. We take care of patients and their immediate family members by providing bereavement counseling up to 1 year postmortem. We assist throughout the healing process and help you move forward. The purpose is to provide emotional, psychosocial, and spiritual support before and after your loved one has passed away.
Respite care is a critical element of hospice care. The physical and emotional stress that a caregiver endures can be unimaginable. Caregivers deserve a well-needed break to help clear their minds and provide rest. We accommodate up to 5 days of respite care for primary caregivers. The patients are transported to another facility where they can be monitored 24/7 and have their own mini-vacation.
