Part of the recipe for our success is the team that helped put it all together. Transition Care started with a simple concept – to provide the best patient care for home health and hospice services. We stand by these principles and are dedicated to building an organization that is focused on positive patient outcomes.

Pamela Mangen (CEO & President)

MBA, MSN, RN, PHN, PCCN-CMC

Pamela carries the MBA, MSN, RN, PHN, and PCCN-CMC designations. In her past leadership role, the hospital achieved multiple Press Ganey awards for patient experience and became the 24th happiest place to work in the healthcare industry. She promotes clinical excellence through research and development, and is a Lean Six Sigma Green Belt (LSSGB) advocate.


Michael Flammer (CFO & CCO)

Michael is the head of corporate compliance and finance for Transition Care Telemetry. His role ensures that Transition Care Telemetry follows all applicable laws and regulations so that we are able to provide exceptional care for our patients. In addition. Michael has an extensive background in financial services, insurance, tax strategies, and mergers/acquisitions.


Dr. Kaishauna Guidry, MD, HMDC (Associate Medical Director)

Dr. Kaishauna Guidry is a California based mobile physician. She received her BA from UCLA, majoring in Sociology, completed her medical degree at Texas A&M College of Medicine, and received internal medicine training from Loma Linda University Health. Dr. Guidry owns a mobile practice called “MOURNING DOVE MEDICAL” providing physician care at your home. This is especially crucial for those who have mobility issues getting to a primary care physician. Dr. Guidry provides excellent care for geriatric patients, and patients with mobility difficulties. She is an experienced community palliative care physician with CAPC designation and focuses on home health improvements that are both holistic and traditional medical care. She is an advocate speaker for palliative and hospice care, and holds a special interest for homebound patients at the late stages of life. Dr. Guidry has published several books on hospice care and often releases podcasts that helps empower everyone.


Brian Flammer (CTO and Contract Administrator)

Brian is a UCSD alumni with a Bachelor of Science in Physics. Brian works on research and development for the organization, and strives to bring the latest advancements in technology to improve patient outcomes. Brian also has a financial services background and has obtained his CTS and CES designations.


Graziella Starr (Director of Patient Care)

Graziella is a Registered Nurse and a Nurse Practitioner with over 15 years of nursing in various capacities that include critical care, trauma, medical surgery, and telemetry. Experienced in triage patients, wound care, and intensive care patients providing best of critical care, therapeutic, preventive, and rehabilitation care plan. As the Director of Patient Care, she oversees clinical training, services, and operations. Graziella also ensures that quality assurance and control are met and aligns with CDPH guidelines. She has her BS and MS, concentration in Family Nurse Practitioner.


Maylia Tsen (VP of Marketing and Client Relations)

Maylia has 20+ years of experience consulting for executive teams and business owners to become better leaders, resulting in growth and more profitability. Her primary involvement has been strategic planning, sales, marketing, integration, operational execution, and financial management. She has worked with Fortune 100 companies as well as start-ups that were both domestic and international. She received her MBA in Marketing and International Business from Pace University in New York and has facilitated CEO Board Groups.

Ginger Black (Content Marketing and Blog Writer)

Ginger has 20+ years in human resources and people development, following a career in IT. She has worked with Fortune 500 companies and then later decided to start her own private practice for leadership and people development, helping employers make better hiring decisions. With her practice, she began doing her own blogs to help engage her clients in ways for improvement. Ginger is an advocate writer and creates articles and information that are informative, resourceful, and engaging. Ginger has her MBA from Frostburg State University (part of University of Maryland) and a BS from The University of Delaware.
